Illness/Absence Policy
Each student is allowed three sick days per year. In the event more than three sick days per year are required, vacation days will be utilized. In the event of an illness, fever (temperature > 100.5°F-orally) vomiting/diarrhea, or other absence within 24 hours of an assigned shift, you must notify your Associate Director, preceptor, and unit by phone no later than two hours before your assigned shift begins. You must also send an email explaining your absence to your Associate Director within 24 hours of any absence.

Tardiness Policy
Please make every attempt to be professional with attendance. Punctuality is the expectation for each student. If you are late to an assigned clinical day, you must notify your Associate Director and preceptor by phone immediately. You must also send and email explaining your tardiness to your Associate Director within 24 hours.

Preceptor Call-offs/Illness Policy
You are responsible for establishing professional communication with your preceptor. It is the expectation that preceptors will notify each student prior to any absence. Please contact your Associate Director by phone immediately when you are aware of a predicted absence so alternate arrangements can be made. Observations are assigned when preceptors are ill or are on vacation. They may not be requested to meet your personal needs.

Schedule Policy
Staffing schedules are in 6 week increments, and the final schedules are posted at all three hospitals approximately 2 weeks prior. It is your responsibility to notify your Associate Director with your schedule in an email no later than 1 week prior to when a 6-week schedule begins. In the event of a schedule change, you must notify your Associate Director by phone and in an email at least 24 hours before the change occurs.

Vacation Policy
Each student is allowed 10 vacation days per year. A Vacation Request Form (on your thumb drive) must be electronically submitted to your Associate Director in an email 2-4 weeks prior to your requested dates. Vacation days must be used as a whole day, they may not be split.

Thumb Drive Policy
A thumb drive will be issued to each student participating in the Co-op program, which will be used for the documentation of your progress. Your thumb drive needs to be carried with you at all times during Co-op shifts. It is your responsibility to update the Competency Record and Weekly Progress Record on your thumb drive weekly with your preceptor during summer months and over winter break, and monthly when school is in session. You must email your Weekly Progress Record to your Associate Director weekly during summer months and over winter break, and monthly when school is in session. If the thumb drive is lost or damaged, the student is responsible for paying for the replacement. The thumb drive should be used for Co-op program purposes only!  Storage of files, information, or programs for other purposes is strictly prohibited until the completion of the Co-op program.

Instructions on Dress

Professional Dress

  • Lab coat with nametag
  • No open toe or open heel shoes
  • No holey crocs, please
  • No low cut tops, no bellies showing
  • No blue jeans, jean skirts, or any denim fabric, etc.

Uniform

  • May wear solid white t-shirt under the scrub top (tank, short sleeve or long sleeve
  • CCHMC Co-op students must wear blue CCHMC Co-op shirt with white uniform pants at all times while in clinical
  • Wear white/neutral underwear (the pants tend to be thin)
  • May substitute other uniform pants but make sure they are thick material
  • Uniform/lab coats should look new, not yellowed or worn
  • White shoes must be leather, clogs aren’t allowed, may wear athletic shoes as long as they are all white and leather
  • Must wear school patch on uniform
  • Lab coat may be worn while giving care to patients as long as you are planning to wash it daily like you do the uniform
  • Hair up, not on collar
  • No perfumes
  • Only jewelry - wedding rings
